The Lancet Commission report in 2015 highlighted the role of safe surgery and anaesthesia. They also gave a framework of recommendations, indicators and targets to achieve universal access to safe surgery. This article explores the applicability of these indicators in Pakistan namely; access to timely surgery, specialist surgical workforce diversity, surgical volumes, perioperative mortality rate, protection against impoverishing expenditure and protection against catastrophic expenditure. The objective of this study was to illustrate the statistical and medical characteristics of females younger than 40 years presenting with breast cancer at GINUM. A descriptive case series in which 235 patients who fulfilled the inclusion and exclusion criteria were included. The majority of the patients had advanced disease at presentation. Family history was positive in 59 (25.11%) patients. Patients having grade III and II disease were 142(60.42%) and 89 (37.87%) respectively. Thirteen (5.5%) patients had Right-sided breast cancers in our series. No significant association of null parity with breast cancer was found. Receptor status studies revealed only 2.55% increased oestrogen receptor/progesterone receptor (ER/PR) positive patients as compared to oestrogen receptor/progesterone receptor (ER/PR) negative patients. Our population shares slightly different features as compared to Western population because there were slightly increased right-sided tumours and majority of our patients were parous.Selective tooth agenesis is a congenital disorder divided into two types based on the number of missing teeth, i.e. hypodontia which is the absence of 6 permanent teeth excluding the third molars. Brigimadlin As the prevalence of tooth agenesis is higher in populations with Arab and Asian descent, it is intriguing to probe deeper into the molecular aspects of this disorder. Selective tooth agenesis inherits as autosomal dominant, autosomal recessive or X-linked dominant mode of inheritance. The 10 loci identified are selective tooth agenesis 1 through 9 and selective tooth agenesis X1. Genes for 8 of these loci have been characterised while the causative genes for selective tooth agenesis 2 and 5 still remain to be elucidated. The current broad-spectrum review was planned to discuss the molecular genetics of all 10 loci mapped with selective tooth agenesis, their mode of inheritance as well as the proteins encoded by these genes, their roles and their interactions.Medical educators are required to explore and introduce teaching and learning innovations to enhance students’ cognitive growth and conceptual understanding. It is of particular importance in subjects of basic sciences which encompass basic principles of medical science and link the spheres of core knowledge and clinical application. There are multiple traditional approaches to teaching and learning of anatomy with varied preferences of use. Visualisation holds pivotal importance in learning anatomical knowledge. This review paper proposes use of drawing as a strategy to learn anatomy and describes some conditions for best use of drawing as a learning tool.Medulloblastoma is the most common malignant brain tumour in children and is a major cause of mortality and morbidity, particularly in low- and middle-income countries. It has been risk-stratified on the basis of clinical (age, metastasis and extent of resection) and histological subtypes (classic, desmoplastic and anaplastic). However, recently medulloblastoma has been sub-grouped by using a variety of different genomic approaches, such as gene expression profiling, micro-ribonucleic acid profiling and methylation array into 4 groups, namely Wingless, Sonic hedgehog, Group 3 and Group 4. This new sub-grouping has important therapeutic and prognostic implications. After acute leukaemia, brain tumour is the second most common malignancy in the paediatric age group. The improvement in outcome of acute lymphoblastic leukaemia in low- and middle-income countries reflects the relative simplicity of diagnostic procedures and management. Unlike leukaemia, the management of brain tumours requires a complex multidisciplinary approach, including neuro-radiologists, neurosurgeons with a paediatric expertise, neuropathologists, radiation oncologists and neuro-oncologists. In addition, the equipment required for the diagnosis (magnetic resonance imaging scan, histological, molecular and genetic techniques) and the management (operating room, radiation facilities) is a limiting factor in countries with limited resources. In Pakistan, there are very few centres able to treat children with brain tumours. The current literature review was planned to provide an update on the management of this tumour.

To investigate significant differences, if any, between figure of-eight method and simple wire closure technique in reducing the incidence of infectious and non-infectious sternal dehiscence in patients undergoing sternal closure.

The systematic review was conducted in the Cardiothoracic Surgery Department at Aga Khan University from 1st December 2015 to 13th December 2017. The review was registered with PROSPERO, the International Prospective Register of Systematic Reviews. Pubmed, Cochrane Library (Wiley) and Scopus databases were searched for articles published up to December 13, 2017. The search was limited to randomised control trials, clinical trials, retrospective cohort studies, journal analyses, systematic reviews and meta analyses. Cadaveric or animal studies and articles published in languages other than English were excluded.

Of the 286 articles retrieved, 265(92.6%) were excluded on the basis of study title and abstract. Another 15(5.2%) were excluded for being irrelevant to the topic n hand, and 6(2.1%) formed the final sample. Of them, 4(66.6%) studies showed no significant difference between the two techniques, while 2(33.3%) found figure-of-eight technique to be superior of the two.

There was no significant difference between the figure-of eight method and the simple wire technique in reducing the risk of dehiscence in patients undergoing sternal closure.
