Among primary malignant bone tumors, osteosarcoma reigns supreme in frequency. In a single cancer center in Tianjin, China, this study assessed the overall health of primary osteosarcoma patients and explored the contributing factors in osteosarcoma cases exhibiting lung metastasis.
From February 2009 to October 2020, a retrospective evaluation was undertaken of patients from Tianjin Medical University Cancer Institute and Hospital located in China. In order to assess the overall survival rates of osteosarcoma patients, the Kaplan-Meier method was applied. The prognostic factors for osteosarcoma patients were evaluated through a Cox proportional hazards regression analysis, considering separately the patient population and those with lung metastasis. Correspondingly, the factors influencing the occurrence of lung metastasis were found in patients with concurrent lung metastasis (SLM) and patients presenting with metachronous lung metastasis (MLM).
A total of two hundred three patients participated in the study; 150 patients had their survival status successfully tracked. The survival rate for osteosarcoma over five years reached a significant 700%, whereas the survival months for SLM and MLM patients totalled 333,126 and 458,740, respectively. For osteosarcoma, the presence of lung metastasis was identified as an independent prognostic indicator. In osteosarcoma patients, subsequent development of lung metastases was observed in 21 (103%) cases where lung metastasis was present at the initial diagnosis, and 67 (33%) cases where metastasis emerged during the disease’s later course. The development of SLM was linked to T3 stage (odds ratio 11415, 95% CI 1362-95677, p=0.0025) and bone metastasis (odds ratio 6437, 95% CI 169-2451, p=0.0006). Elevated Ki-67 (OR=2958, 95%CI 1098-7969, P=0.0032), along with elevated LDH (OR=1791, 95%CI 1020-3146, P=0.0043), bone metastasis (OR=1842, 95%CI 1053-3224, P=0.0032), and substantial necrosis (90%, OR=0.0032, 95%CI 0.0050-0.0412, P<0.0001), were identified as independent risk factors for developing MLM.

The laparotomy was undertaken with awake opioid-based thoracic epidural anesthesia as the sole anesthetic agent. This case report, the first of its kind, describes the successful use of awake laparotomy for significant abdominal surgery, resulting in an excellent feto-maternal outcome.

Rude, dismissive, or aggressive behavior, when occurring in a workplace, represents incivility. Instances of incivility are escalating within the healthcare system, leaving minority groups vulnerable. Medical students’ experience of incivility, particularly during clinical placements, can significantly affect their academic learning and mental wellness. Thus, this exploration investigated the viewpoints and realities of discourteous behaviors experienced by medical students from healthcare workers.
Thematic analysis of a qualitative online questionnaire study is detailed in this work. The recruitment of students in years 3 to 5, or those completing an intercalated year, at a significant West Midlands medical school, took place between May and June 2022. Qualitative, free-text questions probed students’ personal experiences with incivility displayed by healthcare professionals during the last 12 months, as well as their corresponding responses and actions.
