    Hospital access limitations are driving the troubling trend of extended wait times in Canadian emergency departments (EDs). Quantifying the effect of alternative care levels on hospital access blockades and estimating the possible consequences of various interventions on emergency department wait times was our objective.

    Six Canadian hospitals employed discrete-event simulation models to emulate patient movement patterns in their emergency departments and acute care settings. The model was filled with administrative data acquired from various sources, covering the period from April 2017 to March 2018. Six intervention strategies were modeled to measure their impact on three key outcomes: (1) the time taken for an initial physician evaluation, (2) the time taken for inpatient bed assignment, and (3) patient attrition from the system due to premature departure.
