Purpose Exclusive breastfeeding is an integral component of Kangaroo Mother Care (KMC). However, the practice of breastfeeding in KMC position is often suboptimal. Hence, a Quality Improvement (QI) initiative study was conducted to improve breastfeeding rates while providing KMC. Materials and Methods Fish bone analysis was used to identify the potential barriers, which were targeted to bring improvement through Plan-Do-Study-Action (PDSA) cycles. Eligible mother-infant (≥34 weeks) dyad who were admitted in Neonatal intensive care unit during the study period were enrolled in the study (n = 45). QI was implemented through two PDSA cycles. In the first PDSA cycle, training and sensitization of health care providers was done. In the second PDSA cycle, mothers were educated and trained for breastfeeding in the KMC position. Data were collected using bed side nursing charts and interviewing the mothers. Data were analyzed using run charts and SPSS software. A p-value of less then 0.05 was considered to be significant. Results Percentage of mothers practicing breastfeeding in KMC position increased to 50% after first PDSA cycle and to 100% after the second PDSA cycle from the baseline of less then 10%. Average duration of KMC increased significantly from baseline 6.09 to 10.9 hours (p = 0.003) in first cycle and 15.6 hours in second cycle (p less then 0.001). Conclusion QI measures increased the rates of breastfeeding in KMC position. The total duration of KMC per day was also significantly increased.Purpose To evaluate the longitudinal changes in retinal vessel diameters in patients with coronavirus disease 2019 (COVID-19).Methods This study included 25 patients with COVID-19 (Group 1) and 25 healthy subjects (Group 2). The diameters of peripapillary temporal and nasal retinal arteries and veins were measured at baseline and at 4 months after remission.Results The baseline diameters of the inferior temporal vein and the artery were increased in group 1 compared to controls (p = .007 and p = .041, respectively). There was also an increase in the diameters of the inferior and superior nasal veins and arteries in group 1 at baseline (p = .001, p = .019, p = .037, and p = .008, respectively). Retinal vessel diameters decreased after remission in all quadrants in comparison to baseline measurements (all p less then .05).Conclusion Increased retinal vessel diameters were measured in COVID-19 patients during the disease. Measurement of retinal vessel diameters may be a noninvasive method of estimating the vascular risk.Basidiomycete fungi eject basidiospores using a surface tension catapult. A fluid drop forms at the base of each spore and, after reaching a critical size, coalesces with the spore and launches it from the gill surface. It has long been hypothesized that basidiomycete fungi pack the maximum number of spores into a minimal investment of biomass. Building on a nascent understanding of the physics underpinning the surface tension catapult, we modeled a spore’s trajectory away from a basidium and demonstrated that to achieve maximum packing the size of the fluid drop, the size of the spore, and the distance between gills must be finely coordinated. To compare the model with data, we measured spore and gill morphologies from wild mushrooms and compared measurements with the model. The empirical data suggest that in order to pack the maximum number of spores into the least amount of biomass, the size of Buller’s drop should be smaller but comparable to the spore size. Previously published data of Buller’s drop and spore sizes support our hypothesis and also suggest a linear scaling between spore radius and Buller’s drop radius. Morphological features of the surface tension catapult appear tightly regulated to enable maximum packing of spores. If mushrooms are maximally packed and Buller’s drop radii scale linearly with spore radii, we predict that intergill distance should be proportional to spore radius to the power 3/2.
Inguinodynia after inguinal meshplasty is a notable complication with an incidence of 13-37%. We wanted to determine if a perineural infiltration of bupivacaine given intraoperatively would reduce the incidence of immediate postoperative pain and inguinodynia.
We have conducted a single-blinded randomized controlled trial including 100 patients with inguinal hernia. Patients in the control group received only spinal anaesthesia. The intervention group received perineural bupivacaine infiltration in addition to spinal anaesthesia. Intraoperatively Ilioinguinal, iliohypogastric, and genital branch of the genitofemoral nerve was identified and 2 mL of 0.5% bupivacaine was given perineurally along these identified nerves. Presence of inguinodynia, immediate post-operative pain scores using the Wong-Baker faces pain scale, duration of analgesics use, use of additional analgesics along with Paracetamol 500 mg tablet, and the length of hospital stay between the groups were recorded.

Motorcyclists are frequently involved in road traffic accidents and may suffer polytraumatic injuries, contributing a substantial burden on healthcare systems particularly in lower-middle-income countries. This study aimed to describe patterns of injury, in-hospital course, and outcomes of road accident trauma in motorcyclists, including polytrauma, at a Trauma Facility in Pakistan.
A retrospective review was conducted by using relevant trauma codes to extract data from records for all victims presenting with motorcycle trauma between January 2018 and June 2019, to a tertiary care hospital in Pakistan. Data collected included victim characteristics, mechanism of accident, patterns of traumatic injury, management, in-hospital complications, and outcomes. Polytrauma was defined as traumatic injuries in two or more anatomic regions with abbreviated injury scale (AIS) ≥ 3. Multivariable logistic regression, adjusted for age, gender, and mechanism of accident, was performed to identify in-hospital complications and outcomes associated with polytrauma.
