It may be possible to re-initiate lenvatinib treatment with good efficacy against distant metastatic lesions, thereby contributing to long-term survival.Gallbladder cancer (GBC) is one of the most frequently observed cancers in India that is usually diagnosed at an advanced stage. Although surgery remains the only curative option, the majority of GBCs are unresectable. Palliative chemotherapy with gemcitabine and cisplatin is the recommended treatment in such cases. The current study reports a case of a 47-year-old female who exhibited GBC that had metastasized to the liver and peritoneum. She was administered palliative chemotherapy with gemcitabine and cisplatin, but due to disease progression the regimen was changed and an aggressive treatment initiated with gemcitabine and oxaliplatin with additional biosimilar bevacizumab (modified Gemox-B regimen). The patient completed six chemotherapy cycles with partial response and received bevacizumab (7.5 mg/kg 3-weekly) based maintenance treatment for an additional 6 cycles, after which she demonstrated disease progression, thus having a progression free survival of ~11 months. The patient is currently receiving palliative chemotherapy with capecitabine.
Immunization is an important strategy for controlling the COVID-19 pandemic. COVID-19 vaccination was recently launched in Uganda, with prioritization to healthcare workers and high-risk individuals. In this study, we aimed to determine the acceptability of COVID-19 vaccine among persons at high risk of COVID-19 morbidity and mortality in Uganda.
Between 29 March and 14 April 2021, we conducted a cross-sectional survey consecutively recruiting persons at high risk of severe COVID-19 (diabetes mellitus, HIV and cardiovascular disease) attending Kiruddu National Referral Hospital outpatient clinics. A trained research nurse administered a semi-structured questionnaire assessing demographics, COVID-19 vaccine related attitudes and acceptability. Descriptive statistics, bivariate and multivariable analyses were performed using STATA 16.
A total of 317 participants with a mean age 51.5 ± 14.1 years were recruited. Of this, 184 (60.5%) were female. Overall, 216 (70.1%) participants were willing to accept the omparable to the global COVID-19 vaccine acceptance rate. Increased sensitization, myth busting and utilization of opinion leaders to encourage vaccine acceptability is recommended. The outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) from a seafood market in Wuhan, China, has ushered in a new era. Antiviral inhibitor It transformed into a pandemic, seized global attention, and was the biggest highlight of the year 2020. The SARS-CoV-2 outbreak has jeopardized health systems and greatly affected socioeconomic parameters. With global focus on fighting this unpredictable fight with this new virus, the biggest chronic infectious killer, mycobacterium tuberculosis (M. tb), was hugely affected from this shift in attention. Due to certain similarities in the behavior of the two infectious agents, there have been inevitable consequences. On one hand, administrative measures to contain SARS-CoV-2 have simultaneously led to a breaking in the chain of tuberculosis (TB) management. Consequently, a regression occurred in the milestones achieved in the battle against TB. On the other hand, the same measures and heightened hygiene awareness has helped to decrease the spread of the TB bacilli. With an improved understanding of the interrelations and the outcomes noticed in 2020, we can better gear ourselves to develop a more sophisticated and robust strategy to tilt the balance against TB. Keeping this in mind, in this review we aim to discuss in detail the implications of SARS-CoV-2 on an already unwavering health hazard TB.Thyrotoxicosis-induced cardiomyopathy is a rare complication occurring in less then 1% of the population, which can require mechanical circulatory support (VA-ECMO) as a bridge to anti-thyroid therapies. Therapeutic plasma exchange (TPE) is an alternative treatment used to rapidly reduce thyroid hormone levels in refractory cases of thyrotoxic crisis without clinical improvement from other therapies. We describe a novel technique of facilitating plasmapheresis via a VA-ECMO circuit in a 26-year-old man with thyroid storm and subsequent circulatory collapse.
The Oropharyngeal squamous cell carcinoma is the second most common head and neck malignancy. Bcl-2 expression alterations have been reported invariably in different cancers. It plays a part in carcinogenesis by inhibiting programmed cell death and thus increasing cell survival.
The present study was conducted in Department of Pathology, S.G.T. Medical College and University, Gurugram over a period of one year (2019-20) on biopsy proven cases of squmaous cell carcinoma of oropharynx. Grading of the tumor was done using Anneroth’s multifactorial grading system. The Bcl-2 scoring was done.
In the present study, a total of 75 cases of oropharyngeal SCC constituted the study group, with the mean age at presentation of 56.63 years. The correlation between Anneroth’s grading system and WHO grade was found to be statistically significant, while correlation between WHO grade with lymph node status was found to be statistically non significant.
There was significant correlation between Anneroth’s grading system and WHO grading system in SCC of oropharynx and it was found to be more relevant in predicting the stage of tumor. Bcl-2 expression did not correlate with the grading of tumor.

Infertility is inadequately acknowledged as a problem in developing countries. Thyroid Autoimmunity (TAI) has been neatly investigated for its association with unfavorable pregnancy and reproductive consequences. We aim to evaluate Clinical Pregnancy Rate (CPR) as a primary outcome following In Vitro Fertilization/Intra Cytoplasmic Sperm Injection (IVF/ICSI) in women with Thyroid Autoimmunity (TAI).
A Retrospective cohort study included 584 women who underwent IVF/ICSI treatment between November 2012 and April 2017 in Orient Hospital, Damascus, Syria. Patients were tested for TAI before IVF/ICSI procedure.
CPR did not significantly differ between TAI positive and TAI negative groups (p>0.05). Subgroup analysis for only primary infertility patients showed a statistically significant difference in CPR between TAI positive and TAI negative groups.
Although several arguments were in favor of the relation between IVF/ICSI outcomes and Thyroid autoimmune disease, the presence of TAI positivity did not adversely affect the clinical pregnancy rate.
